- Each form must be hand-written, as typed forms will not be accepted.
- The assignment should be submitted to the Canvas assignment drop box.
- The number of drugs per week varies. As some drugs have multiple uses, they may be listed more than once.
- Students should list the appropriate uses and information that is relevant for that week.
