MAP-IT stands for Mobilize, Assess, Plan, Implement and Track and is a proven framework for community health care initiatives.
Familiarize yourself with all the steps of the process, but take special note of the “assess” section.
Step 2: Identify a specific priority for your community relating to the social determinants of health
The “Dig Deeper: Getting at the Roots of the Issue…” portion of the “Assess” section may be of particular assistance here.
Approach this as if you were in charge of implementing a community health care program (using the MAP-IT) framework.
Step 3: In a written response, address the following:
Where is the community you are referencing? Describe your community in detail.
Include information about infrastructure and services, environment, economic opportunity, safety and security, housing, access to healthcare.
What is the community healthcare priority you have identified?
Which of the social determinants of health is most related to your healthcare priority?
As an RN, what opportunities/solutions would you propose to address this priority and the associated social determinants of health implications?
